Mid-Michigan Community Health Access Program

The Mid-Michigan Community Health Access Program (CHAP) is a community-based, family-centered medical home improvement program that works to improve the health outcomes of children and adults. Mid-Michigan CHAP is a free program that is available to anyone in need, regardless of age or insurance status.
