CHAP Healthy Heart Ambassador Program
The Mid-Michigan Community Health Access Program offers a Healthy Heart Ambassador Program, which teaches individuals about blood pressure self-monitoring and making healthy lifestyle choices. To get involved, ask your PCP for a CHAP referral today!

Commit to Fit Resource Calendars
Commit to Fit is dedicated to making sure the community is aware of the resources available to them! Our team shares monthly community-wide fitness and nutrition calendars to make sure our community is empowered to make healthy choices.

FHEO's Digital Literacy Program
Recently, 20 FHEO program participants received laptops for completing digital literacy training. The laptops will benefit participants as they enter into healthcare training and begin applying for employment. This training was made possible in partnership with Comcast’s Project Up.

R.I.S.E. Program
The R.I.S.E. team, comprised of multiple community-based organizations, providers, and agencies, seeks to provide a comprehensive, holistic set of services including mental health support, peer recovery coaching, and care navigation for social determinants of health, based on each individual’s needs. These services are offered to individuals with mild to moderate charges being lodged at the Genesee County Jail while they are incarcerated and for up to 6 months upon re-entry. In addition to serving justice-involved individuals, the R.I.S.E. team will also serve their household to help the family heal together and increase the likelihood of long-term success for the incarcerated individual. R.I.S.E. collectively seeks to reduce recidiviscm, increase entry and adherence to treatment rates, and improve long-term health outcomes of justice involved individuals and families.
