Mid-Michigan CHAP Steering Committee

The Mid-Michigan CHAP Steering Committee seeks to implement a collaborative, community-based medical home model to improve the health outcomes of children and adults insured by Medicaid, while raising the quality of care, increasing access to care, better utilizing resources, and decreasing costs. The Steering Committee recognizes that a medical home is primary care that is accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective.

The CHAP Steering Committee guides the design and implementation of the CHAP process via strategies that transform health service delivery at the patient, provider, and system levels. It develops best practices and the infrastructure necessary for Michigan pediatricians and other healthcare providers to implement the CHAP model. It is charged with evaluating the effectiveness of the CHAP model and developing a business plan that will lead to program sustainability and expansion throughout Mid-Michigan’s provider network. The CHAP Steering Committee also advocates for system-level improvements in the delivery of health care in our community.

The CHAP Steering Committee has convened medical provider representatives from pediatric and family medicine practices, private medical practices, federally qualified health centers, and safety-net providers with those from behavioral health, public health, human services, community services, and education in this collaborative effort.
