The Community Care Hub Steering Committee seeks to connect residents to a medical home, support a cross-sector screening and referral strategy to address social vulnerability and resource needs of residents, integrate healthcare and social/community service delivery, and ensure effective linkages to needed health services. The Community Care Hub Steering Committee’s priority population is Mid-Michigan children and adults covered by both private and Medicaid insurance, as well as those residents who are uninsured and underinsured.
