Community Health Needs Assessment Project

As part of the Affordable Care Act, all non-profit hospitals are required to conduct an annual community health needs assessment (CHNA) – an ongoing process to evaluate the health needs of the community, prioritize the needs identified, and develop/implement strategies to address them. Genesee County’s three hospitals (Ascension Genesys Hospital, Hurley Medical Center, and McLaren Flint) along with the Genesee County Health Department have agreed to work together and have the GFHC facilitate a joint CHNA for their organizations as well as coordinate implementation plans for meeting community needs. The initial joint Community Health Needs Assessment for Flint & Genesee County was completed in October, 2012.

To assist with this process, the GFHC utilizes its existing Community Data Scorecard and partner relationships, and conducts resident input surveys to ensure needs are community informed.
